Temporary Social Work Services

Include OBRA/PAS screenings, admission notes, psychosocial histories, mini mental status exams, Beck Depression Inventory and Geriatric Depression Scale assessments, MDS/PPS documentation, RAPs, care plans, quarterly reviews, family meetings, LICSW supervision, discharge planning, advance directives, staff training on resident rights, Resident & Family Council, and referrals for behavioral health, elder law, hospice, Medicaid planning, and post-discharge care.

Professional Development Services

The New Social Worker Start Up plan includes two parts: three 1-day sessions specifically focused on orientation to the extended care environment, the social worker's role on the interdisciplinary team, the medical record as communication tool, the PPS and MDS, the survey process, ethics, and community resources.  The second part is a choice of two half-day sessions from the following options: how to facilitate effective family meetings, how to troubleshoot complex psychosocial problems, how to promote positive nursing-social work relationships, how to set up an organized office, and how to support and advocate for residents.

Geriatric Care Assessment Services

GCA services focus on a comprehensive assessment of the older adult's specific needs.  These may include home health care, nutrition and meal preparation, homemaking and housecleaning, moving, personal emergency response systems, transportation, individual supportive psychotherapy, medication management, lab work, estate planning, advance directives, outpatient therapy, companionship services, follow-up appointments, end of life care, and insurance issues.
